This Cookie Policy explains how Dr. Phelps LLC (“Dr. Phelps LLC,” “we,” “us,” or “our”) uses cookies and similar technologies when you visit dr-phelps.com (the “Site”) and related pages hosted on app.jenncoles.com that we use through the LeadConnector Platform.
This Policy should be read together with our Privacy Policy, which explains how we collect, use, and protect personal information more broadly.
By continuing to use the Site after seeing our cookie notice, you consent to the use of cookies as described in this Policy, subject to your choices and browser settings.
Cookies are small text files that are placed on your computer or mobile device when you visit a website. They are widely used to make websites work, improve user experience, and provide information to website owners.
Similar technologies (such as pixels, tags, scripts, and local storage) may also be used for similar purposes. In this Policy, we use “cookies” to refer to both cookies and similar technologies.
Texas does not currently have a stand-alone “cookie law” like some U.S. states or the European Union. However, we use cookies in a way that is consistent with:
Texas Deceptive Trade Practices-Consumer Protection Act (DTPA) principles (avoiding deceptive or misleading practices);
General U.S. privacy and consumer protection expectations; and
Other applicable data protection laws that may apply to visitors from outside Texas or the U.S.
We aim to provide clear, transparent information about the cookies we use and give you meaningful choices about how your information is collected and used.
We use a combination of first-party cookies (set by our Site) and third-party cookies (set by other domains, such as analytics providers and our LeadConnector Platform).
These cookies are necessary for the Site and our forms to function properly. They are usually set in response to actions you take, such as:
Navigating between pages;
Submitting contact or scheduling forms;
Managing basic security and performance;
Remembering limited choices related to cookie consent or form state.
Without these cookies, parts of the Site and embedded forms may not work correctly. Because they are essential, you cannot opt out of these cookies via our on-site tools, but you may still block them in your browser (which may break functionality).
Examples include:
Basic session cookies for dr-phelps.com;
LeadConnector/LeadConnector-hosted page cookies on app.jenncoles.com that allow forms and scheduling tools to function;
Security or load-balancing cookies set by our hosting provider.
We use analytics cookies to understand how visitors use the Site so we can improve content, usability, and overall performance.
We primarily use:
Google Analytics
Helps us understand which pages are visited, how long visitors stay, and how they arrive at the Site.
Google Analytics may set cookies that collect information such as IP address (often truncated or anonymized), browser type, pages visited, and time spent on pages.
We use this information in aggregated form and do not use Google Analytics to identify you personally.
Google Tag Manager
Google Tag Manager itself is generally used to load and manage other tags (such as Google Analytics). It may collect limited data necessary to ensure tags function correctly.
Where required by law for certain visitors (for example, in the EU/UK), these cookies may only be used after consent. For most Texas and U.S.-based visitors, we rely on legitimate interests in understanding how our site is used, while still offering opt-out options (see Section 6).
We use LeadConnector (domain app.jenncoles.com) to host some forms, landing pages, and scheduling tools.
When you interact with those pages, LeadConnector may set cookies to:
Maintain session state;
Process form submissions and appointment requests;
Perform basic analytics about how those pages are used.
These cookies are subject to LeadConnector’s own policies in addition to this Policy. We do not control all aspects of how LeadConnector or other third parties use their cookies, but we choose providers that are widely used and provide reasonable transparency.
You may visit the privacy and cookie information pages of these third parties (such as Google and LeadConnector) for more detail on their practices.
Depending on the type of cookie, information collected may include:
Device and browser information (type, version, operating system);
IP address and general location (e.g., city/region);
Pages visited, links clicked, and time spent on the Site;
Referring URLs (how you arrived at the Site);
Information you provide in forms (for example, to keep your place in a form or remember certain choices).
We use this information in accordance with our Privacy Policy and do not sell personal information.
You have several options to control or limit cookies on your device.
Most browsers allow you to:
Block all cookies;
Block cookies from specific sites;
Clear or delete cookies when you close the browser;
Receive notifications before a cookie is stored.
Please refer to your browser’s help section for specific instructions. Keep in mind that if you block or delete cookies, some features of the Site (especially forms and scheduling tools) may not work properly.
We display a cookie banner or preference tool on the Site, you are be able to:
Accept all cookies;
Accept only essential cookies;
Adjust settings for different categories of cookies (where available).
Your choices may be stored in a cookie so the Site can remember your preferences.
You can also control certain analytics cookies by:
Installing the Google Analytics Opt-out Browser Add-on, available from Google;
Adjusting ad personalization and privacy settings in your Google account (if you have one).
These tools are provided and controlled by Google and may change over time.
Some browsers offer a “Do Not Track” (DNT) signal that can be sent to websites. There is currently no widely accepted standard for how websites should respond to DNT signals, and we do not currently respond to them.
You can still use the other methods described above to control cookies and tracking.
Information collected via cookies is generally kept for as long as necessary to fulfill the purposes described in this Policy, including:
Site operation and security;
Analytics and performance;
Legal and regulatory compliance.
Analytics cookies may have their own storage duration (for example, Google Analytics cookies typically persist for several months to two years, depending on settings). Aggregated analytics data may be kept longer, but is not used to identify individual users.
Our Site is not directed to children under 13, and we do not knowingly use cookies to collect personal information directly from children under 13.
If you believe a child under 13 is using the Site and providing personal information, please contact us so we can review and, where appropriate, delete the information consistent with applicable law.
We may update this Cookie Policy from time to time to reflect:
Changes in the cookies and technologies we use;
Changes in our service providers (such as analytics or LeadConnector);
Changes in applicable law or guidance.
When we make changes, we will update the “Last Updated” date at the top of this Policy. In some cases, we may also provide additional notice (for example, via a banner on the Site). Your continued use of the Site after changes are posted constitutes your acceptance of the updated Policy.
